CAGE Code (Commercial and Government Entity): is a five character code created by the Central Contractor Registry (CCR) and used by organizations applying for DoD and NASA awards. Organizations must have a separate CAGE Code for each physical location or separate division at the same physical location.

Effective 1 July 1999, SELF CERTIFICATION IS NO LONGER ACCEPTABLE for Small is advantaged Businesses. You must apply for Small Disadvantaged status through your local Small Business Administration office or one of their approved vendors. Applications can be found on the SBA website www.sba.gov and follow the links to Small Disadvantaged Business. You may also request a copy by dialing 202-619-1850 or write to U.S. SBA, Office of Small Disadvantaged Business Certification & Eligibility, 409 3rd Street, SW, Washington, DC 20416. Service Disabled Veteran-Owned - needs to be SBA certified.
SELF CERTIFICATION IS NOT ACCEPTABLE. To be considered a Service-Disabled Veteran, the veteran must have an adjudication letter from the Veterans Administration (VA), a Department of Defense Form 214, Certificate of Release or Discharge from Active Duty, or a Statement of Self-explanatory. Signature, Title and Date – HOW TO SIGN AND SUBMIT THIS FORM
1. In the Authorized Signature field please type your full name (a middle initial may be used in lieu of your entire middle name. If you do not have a middle name simply type your first and last name). By typing your name in this field you are agreeing to the terms and conditions of ManTech’s Business Classification form, and confirming that you have supplied all of the information in this Business Classification Form and further, that the data provided in this form is true, correct, up to date and complete, and that you will continue to keep all of the information provided correct, up-to-date and complete;
(a) To the best of your knowledge and belief, neither the information you have provided in this registration nor the manner in which it is directly or indirectly to be used infringes upon the legal rights of a third party;
(b) That the information you have provided will never at any time whatsoever be used for any unlawful purpose whatsoever;
(c) you have the requisite power and authority to enter into this Business Classification form and to perform the obligations hereunder.
You are also advised that the U.S. Government may impose a penalty against a firm misrepresenting its business size and/or disadvantaged status. Eligibility as a small business is based on the regulations issued by the Small Business Administration in CFR 13, Part 121 of the SBA Rules and Regulations.
2. Once you type your name into the first field in the second field you input your title with the rank at your small business. If you are unsure of your title you may leave this field blank
3. In the Name Printed field you must again type in your name. Just as you typed your name in the Authorized Signature field you must do the same here. (It may be redundant however as with all business dealing signatures ensure that each party is voluntarily agreeing to the terms and conditions of the business agreement.
4. In the date field please type in today’s date (the date that it is on the day you fill out and submit the form). Currently there is no format requirement, simply type in the date in any US or European standard format.
